Prehospital thoracostomy.

چکیده
We read with interest the article on prehospital thoracostomy published in the journal [1] and the correspondence that followed recently [2,3]. We work on the unit where the intervention was originally described [4]. In the last 10 years 1079 thoracostomies have been performed on prehospital patients in our system. This amounts to 9% of trauma patients attended in the same period. We are aware that the technique has been adopted by a number of other prehospital services, but the experience of Massaurutti et al. is a rare publication on the subject. We would like to add to the discussion with the following points:

Prehospital chest tube thoracostomy: effective treatment or additional trauma?
BACKGROUND The use of prehospital chest tube thoracostomy (TT) remains controversial because of presumed increased complication risks. This study analyzed infectious complication rates for physician-performed prehospital and emergency department (ED) TT. METHODS Over a 40-month period, all consecutive trauma patients with TT performed by the flight physician at the accident scene were compare...
متن کاملSimple thoracostomy in prehospital trauma management is safe and effective: a 2-year experience by helicopter emergency medical crews.
OBJECTIVE To evaluate the effectiveness and potential complications of simple thoracostomy, as first described by Deakin, as a method for prehospital treatment of traumatic pneumothorax. METHODS Prospective observational study of all severe trauma patients rescued by our Regional Helicopter Emergency Medical Service and treated with on-scene simple thoracostomy, over a period of 25 months, fr...
متن کاملSimple thoracostomy avoids chest drain insertion in prehospital trauma.
Tension pneumothorax and hemothorax are life-threatening emergencies that require immediate treatment. Field stabilization of trauma patients often requires rapid surgical drainage of these injuries but inevitably delays departure for hospital. Conventional treatment involves the insertion of a chest drain but we describe a modified technique of simple thoracostomy that is faster and simpler to...
متن کاملSufficient catheter length for pneumothorax needle decompression: a meta-analysis.
INTRODUCTION Needle thoracostomy is the prehospital treatment for tension pneumothorax. Sufficient catheter length is necessary for procedural success. The authors of this study determined minimum catheter length needed for procedural success on a percentile basis. METHODS A meta-analysis of existing studies was conducted. A Medline search was performed using the search terms: needle decompre...
متن کاملChest decompression during the resuscitation of patients in prehospital traumatic cardiac arrest.
BACKGROUND Tension and bilateral pneumothorax can cause or contribute to death following trauma. A surgical incision (thoracostomy) or needle decompression through the chest wall rapidly treats these conditions. Resuscitation of patients in traumatic cardiac arrest focuses on treating common and reversible life-threatening conditions. A study was undertaken to observe the practice, findings and...
